New Triage Pathway for Adult ASD/ADHD Referrals

Following consultation with the Local Medical Committee (LMC), Derby and Derbyshire Integrated Care Board (DDICB) is launching a new triage pathway for Adult (16+) ASD/ADHD diagnostic assessment referrals.

This started on 1 June 2026 and will require all new ADULT referrals to be triaged by the DDICB. 

Following triage, any accepted referrals will be sent on to the chosen ‘Right to Choose’ provider  Any patient’s referral that has not been accepted for onward referral will be returned to the GP and the patient will be informed.

Please note any Children and Young Persons referral pathways are unchanged.

Proceeding with your referral…

• If you wish to proceed with your ADHD / ASD referral, please choose an NHS Right to Choose provider from the link below.
• You will also need to complete a Patient Questionnaire which will accompany your triage referral.  Please print off a copy from the link below, complete and return to the surgery (or collect a copy from the surgery reception)
• Book a 10 minute appointment, either in person or over the telephone with your GP to discuss the referral.

Once all the information has been gathered together your referral will be sent to the DDICB Triage Service by your GP.
