Fit Notes (sick notes)

If you are unwell and cannot work, you may need a fit note for your employer.

Do you need a fit note?

7 days or less: You do not need a fit note. Self-certify your absence using your employer’s form, if they have one. Count weekends and bank holidays.

If your employer requests medical evidence for the first 7 days, there may be a fee, which your employer should pay.

More than 7 days: You may need a fit note.

How to request a fit note online

Choose administrative request and tell us you need a fit note. We will review your request and contact you if we need more information.

Who can issue your fit note?

A healthcare professional involved in your care can issue a fit note: a doctor, nurse, physiotherapist, occupational therapist, or pharmacist working in a GP surgery, hospital or clinic.

They will assess whether you are not fit for work or may be fit for work with support from your employer.

Returning to work

You can return when you feel able; you do not need an appointment to be signed back as fit for work. If returning before your fit note ends, speak to your employer about any adjustments that could help you return safely.
