Practice Boundary Map and Area Coverage

The Practice usually accepts patients with a home address within the boundary map as detailed.

If you are unsure if your address is in our catchment area, please use the interactive map or make enquiries at the surgery.

When the Practice reaches maximum capacity and we consider that to continue to accept new patients would have a detrimental affect on the care we can provide, we are forced to reduce our Practice boundary to patients who move within a one mile radius of the surgery and do not already have a registered GP in the Chesterfield area.

We will advertise any boundary reduction as and when necessary and apologise if we are unable to accept your registration as a result of these necessary restrictions.

Providing you live within our catchment area you will be able to apply to join the Practice. Please request a New Patient Registration Pack from our reception staff or print out the Registration Forms using the tabbed information above.

You will be assigned a named GP from the date of registration and we will advise you who this is when you collect/return your registration forms. If you move to an area outside our Practice boundary, you will be asked to register at a surgery nearer to your new home.

Please note, if you do not live in our catchment area then we will be unable to register you.
